    Book by phone

    Appointments are arranged by phone. Say that the appointment concerns a patient from a higher infectious-disease risk group. On this number you can arrange a convenient date and find out the available hours.

    Prepare documents before the visit

    Prepare a current certificate from your treating doctor and an identity document with a photo. Without the certificate, eligibility for NFZ-funded care may not be confirmed.

  3. 3

    Come for eligibility assessment and treatment planning

    The dentist reviews the documents, examines the mouth and defines the scope of treatment available under NFZ funding.

What should I bring?

Medical certificate

The certificate is required. Without it, eligibility may not be possible. If you are unsure, call before arriving.

Identity document

Bring an identity card or another photo ID. It helps confirm your details smoothly before the visit.
